What if I have not got a medical card?
The receptionist at the practice will give you a form to fill in (known as a GMS1). You may be asked for proof of your identity. When you have completed and returned the form your new medical card will be sent by post to your home address.
Can I choose my doctor/GP?
A patient no longer registers with an individual doctor / GP but with a practice, you can see any doctor / GP at the practice.
Can I change my practice?
You have the right to change your practice without having to give a reason, but it is always helpful to speak with the Practice Manager first about any concerns you may have.If you move out of the catchment area you may be asked to find another practice.
Can a practice refuse to accept me as a patient?
Yes. You may be refused if you do not live in the practice area or if its list is full.Where a practice does refuse your application, then they must have reasonable grounds for doing so and this must be given to you in writing within 14 days.
GPs have a duty to provide treatment that is both necessary and required immediately to any patient within their practice area.
