Step 3: Registering with a GP

Once you have made a decision about which practice you would like to be a part of, you will need to register with them. 

What will I need to do?

  • Just ring up the surgery or drop in and speak to the receptionist.
  • The practice will ask you to complete a form known as GMS1 which they will hand to you at reception. If you want to fill it in beforehand at home, you can also download it from the NHS Choices website: www.nhs.uk
  • You will also need to show: your medical card (if you have one), proof of identity (passport, driving licence) and a proof of address such as a recent utility bill (gas, electric, phone or council tax bill).
  • If you don’t have, or can’t find your medical card, don’t worry, just advise the practice when you go to register.
  • Once your application has been accepted, they will arrange for your medical notes to be transferred from your previous doctor.

Can a practice refuse to register me?
Yes, you may be refused if you do not live in the practice area or if their patient list is already full. Where a practice does refuse your application, they must have reasonable grounds for doing so, and this must be given to you in writing within 14 days.
